Complete the following form and post it, along with payment, to:

Payment should be either

a cheque in UK pounds, drawn on a UK bank, or
an international bank draft in UK pounds.

The cheque or draft should be made payable to Heriot-Watt University, and labeled "Scottish Functional Programming Workshop" on the reverse. All prices include VAT, 3 days accomodation, all meals, breaks, draft and final proceedings, afternoon outing.

Your form should *arrive* by 20th July 2001. Groups with multiple attendees please complete separate forms.
